To better understand the needs of the community, the Amesbury Council on Aging requests that residents age 55 and older share their views in order to enhance Council on Aging programs and services at the Nicholas J. Costello Transportation Center (The Senior Center).
This survey will take about 10 – 15 minutes. All responses will be kept confidential.
As a thank you for your time, you are invited to enter a raffle to win one of three $50 gift cards!
You will have the opportunity at the end of the survey to enter your information for the raffle, or you may choose to keep your response anonymous.
